NEW PATIENT FORMS
In preparation for your initial evaluation at DORSET PHYSIO or STRATTON PHYSIO, you may print and complete the following Health History & Consent forms and bring them with you.
A new patient welcome packet can be mailed to you at your request.
Please arrive 15 minutes prior to your initial evaluation for registration.
You may be required to complete an outcome measure specific for your condition when you arrive.
Please bring a Photo ID, Insurance Card(s), your medication list, and a form of payment for any Payment Due and/or Supplies. Please wear loose comfortable clothing and supportive shoewear that you would exercise in at each visit.
VISITOR POLICY: We allow a visitor to attend your session only if you require a caregiver to assist you in your care or mobility needs. All others will need to wait outside or in our waiting area. Children and/or siblings are not allowed to attend your visit, so please secure childcare for your appointment. Thank you for your understanding.
