There are many options for your physical therapy needs and we are glad you have chosen us!
AT YOUR FIRST VISIT
Please arrive 15 minutes prior to your initial evaluation for registration and intake paperwork. You may print them out ahead of time (links below) or complete digital versions on your patient portal for your convenience. Please bring your Photo ID, Insurance Card(s), Medication List, and a Form of Payment per our payment policy. Please wear loose comfortable clothing and clean shoe wear that you would exercise in.
VISITOR POLICY
You may bring 1 supportive family member or friend to your appointment if you need assistance in your care. A chaperone will be required for all minors for treatment provided to any sensitive areas for their safety.
